Medicare
- Patients insured under the federal program Medicare will be required to sign certain authorization forms, in which they will request that payment of authorized benefits be made payable to Ross Nathan, M.D. and/or Dr. George A. Macer, M.D., on their behalf, for any services provided to them. By signing, the Medicare patient also authorizes any holder of medical and other information (relating to the patient) to be released to any entity (which may or may not include any insurance company, other Medicare affiliate agents, state medical-assistance agencies, and/or any other governmental or private payer) for the purposes of paying any services provided to the patient.
- Please be advised that our physicians are Medicare providers. As required, our office will submit claims to Medicare on behalf of all qualifying patients. It is our policy to advise Medicare patients of possible non-covered medical services and/or durable medical goods. As part of your formal registration, we disclose this information by signature acknowledgement to all Medicare patients. In the event that such medical services and/or goods are dispensed upon the patient's request, and then denied coverage by Medicare, we will then "balance bill" the patient for any non-covered items. - All unpaid claims, open-account balances, as well as any denial for payment by Medicare is the patient's (or other guarantor's) responsibility. Therefore, the patient (or other guarantor) will be required to pay for all accrued charges until the account is satisfied in full. In addition, the patient (or other guarantor) will assume the responsibility to respond to any correspondence furnished by The Hand & Wrist Center, or billing service, regarding an outstanding account.
